Schreiner Athletic Training Mission Statement
The mission of the Schreiner University Athletic Training program is to provide recognized athletic training services to the student-athletes of Schreiner University. All student-athletes will receive access to injury education, prevention, management and rehabilitation services associated with athletic participation. Referral for additional medical evaluation and treatment as necessary is also provided. The goal is to return any injured student-athlete to activity as quickly and safely as possible.
The Athletic Training staff is dedicated to this mission and compliance to Schreiner University, the Southern Collegiate Athletic Conference, the NCAA, and the National Athletic Trainers' Association.
The Schreiner University Athletic Training program will be under continuous evaluation both internally and externally to insure that quality athletic healthcare is provided to the student-athlete.

Athletic Forms Instructions
Returning Athletes:
1. Please log-in to your ATS account (ATS website: schreiner2.atsusers.com) and click
on athlete information
2. Please update any contact information as needed
3. Update your insurance information/dates/card pictures.
4. Please complete the following forms:
5. 2026-2027 Medical History Form (under “Forms” tab)
6. 2026-2027 Legalities Form (under “Forms” tab)
7. 2026-2027 Medication List (under “Forms" tab)
8. 2026-2027 Returning Athlete Form (under “Forms” tab)
9. Upload Sickle Cell Status (under” Sickle Cell" tab)- If you completed this during a
prior school year, the sickle cell status should already be completed. If it is not:
a. Enter the date of your sickle cell test.
b. Select Negative or Positive
c. Upload a picture of the test results.
d. If you do not have documentation of it your sickle cell status, please follow
the instructions below:
i. If you were born in Texas, please contact your Primary Care Physician
or the hospital where you were born and request your newborn
screening records. You may also be able to get a copy of your results
from the Texas Department of Health Services Newborn Screening
unit.
ii. If you were born out of state, you can contact your Primary Care
Physician or the hospital where you were born and request your
newborn screening records. You may also be able to get a copy of
your results from your states public health agency.
iii. If you were born out internationally or are unable to obtain a copy of
your results, you will have to get a “Sickle Cell Solubility” lab test (via
bloodwork) to determine your sickle cell status. This test can typically
be performed at any doctor’s office or lab facility that does
bloodwork.

Incoming Freshman and Transfer Athletes:
These instructions are for new and incoming student-athletes (1st time freshman,
transfer students, etc.). Please completely read all of these instructions before
continuing. The process may take 45 minutes to 1½ hours to complete.
PARENTS MUST NOT COMPLETE THESE FORMS FOR THE STUDENT ATHLETES.
NOTE: IT IS RECOMMENDED THAT YOU USE A LAPTOP OR DESKTOP COMPUTER TO COMPLETE
THE INFORMATION. USE OF A TABLET/CELLPHON E MAY RESULT IN INCOMPLETE
INFORMATION AND DELAY OF PARTICIPATION.
All Highlighted Boxes on the website are REQUIRED!
1. Go to the following web-site: http://schreiner2.atsusers.com/ and type the following
information and click Log-in.
a. Schreiner ID#: new
b. Password: new
c. (IF NEEDED – Database: atsschreiner)
d. You will have an Athlete ID that will become your log-in and the ability to
create your own password later.
2. You will be at the Athletic Information page. (Please note that the highlighted items
are required to be filled out).
a. Select Team 1: select your main athletic team. If you are participating on
more than one team, select each additional team from Select Team 2 and
Select Team 3
b. Name, Gender, DOB, Phone (Primary), Cell (US numbers only)
c. E-mail: this must be your Schreiner email address: username@schreiner.edu.
You may provide an alternate email address if you don’t know your Schreiner
email address.
d. Primary Address, City, State, and Province/Zip Code/Country: enter your
Home Street/PO Box Number and/or apartment number.
e. Schreiner ID#: You will receive your Schreiner ID number from admissions.
f. Password: Create a password of your choice
g. Year: Newcomer (1st Year Freshman/Transfer_, 2nd Year Returner (Soph), 3rd
Year Returner (Jr), 4th Year Returner (Sr), 5th Year Returners (Sr/Grad)
h. Medical Alerts, & Allergies,: Select from the pull-down menu, click on it, and
wait for it to enter the box, and then you can select another. If you have
none, please type NONE in the box.
i. Click Save Athlete Information …advance to insurance page tab.
3. At the top of the page, Click Insurance tab.
a. Under Company:
i. select your insurance company from the pull-down menu. If your
company is not there, click Add New Insurance Company and add
your insurance company.
ii. If your insurance company is not listed in the pull-down menu and
you cannot add your insurance company please contact the athletic
trainers
iii. Insurance type, ID #, Group #: select the type of insurance and the
insurer’s identification number.
b. Under Policy Holder Information
i. Name, DOB, Gender: type the policyholder’s name, date of birth, and
gender.
ii. Athlete’s Relationship to Insured: from the pull-down menu, select
your relationship to the policyholder (ex. are you the child, self, etc.).
c. Under Upload Insurance Card Images- (please note the instructions in red on
the website) (MAKE SURE THESE ARE EASY TO READ)
i. Front*: scan or take a picture of the front of your insurance card then
upload the image.
ii. Back*: scan or take a picture of the back of your insurance card then
upload the image.
4. Next Click on the Contact tab. This will go to the emergency contacts page.
a. Contact’s Name, Relationship, C ell, E- mail: type your emergency contact’s
name, their relationship to you, their cell number, and their email address.
5. Click on Save Athlete Information on the bottom of the page to save everything.
6. On the next screen, follow the instructions below to complete the forms

FORM 1: MEDICAL HISTORY
1. Under Form Name: select 2026-2027 Medical History from the pull-down menu.
2. Click New and complete the form. This form must be completed and saved prior to
participating in any practice or competition at Schreiner. Read all directions and
answer all questions completely completely.
3. At the bottom of the form, use the mouse to sign your name in the Athlete/Student
Signature box then type your name in the Signed By and click Sign.
4. If necessary, at the bottom of the form use the mouse for your parents/guardian’s
5. to sign their name in the Parent/Guardian Signature box then type their name in the
Signed By box and click Sign.
6. After you have signed the form click Save at the bottom.

FORM 2: LEGALITIES FORM
1. Under Form Name: select 2026-2027 Legalities form from the pull-down menu.
2. Click New and complete the form. This form must be completed and saved prior to
participating in any practice or competition at Schreiner Read and answer all
directions completely. At the bottom of the form, use the mouse to sign your name
in the Athlete/Student Signature box then type your name in the Signed By and click
Sign.
3. If necessary, at the bottom of the form use the mouse for your parents/guardian to
sign their name in the Parent/Guardian Signature box then type their name in the
Signed By box and click Sign.
4. After you have signed the form, click Save at the bottom.
FORM 3: MEDICATION LIST FORM
1. Under Form Name: select 2026-2027 Medication List form from the pull-down menu.
2. Click New and complete the form. This form must be completed and saved prior to
participation beginning after August 1st. Read and answer all directions completely.
At the bottom of the form, use the mouse to sign your name in the Athlete/Student
Signature box then type your name in the Signed By and click Sign.
3. If necessary, at the bottom of the form use the mouse for your parents/guardian to
sign their name in the Parent/Guardian Signature box then type their name in the
Signed By box and click Sign.
4. After you have signed the form, click Save at the bottom.
FORM 5: Sickle Cell Status
1. Click on the Sickle Tell tab
2. Fill out the date of your test
3. Fill out the results of your test- Negative or Positive
4. Take a picture of your sickle cell status (“Sickle Cell Trait”)
5. If you do not have documentation of it your sickle cell status, please follow the
instructions below:
a. If you were born in Texas, please contact your Primary Care Physician or the

You may also be able to get a copy of your results from the Texas
Department of Health Services Newborn Screening unit.
b. If you were born out of state, you can contact your Primary Care Physician or
the hospital where you were born and request your newborn screening

c. If you were born out internationally or are unable to obtain a copy of your
results, you will have to get a “Sickle Cell Solubility” lab test (via bloodwork)
to determine your sickle cell status. This test can typically be performed at

6. Then follow steps 1 through 4 to complete the form.

All forms will be filled out through the ATS portal, use the 2nd link below. Use the 1st link for the step-by-step instructions on how to complete each form.
Returning athletes your "Athlete ID" is your Schreiner ID and then your password.
New Athletes, use “New” as the Athlete ID and “New” as the password

PHYSICALS:
NCAA rules require all newcomer athletes to get a physical before participating in sports.
Schreiner will provide FREE physicals from our team physicians during your on-campus move-in/check-in days.
